AS ABOVE MENTIONED FOSTERER, BY SIGNING THIS CONTRACT, I AGREE TO AND

UNDERSTAND THE CONDITIONS SET FORTH.

1. Only altered or to be altered pets will be fostered from HHAF

2. No animals are to be bred or allowed to breed, this may result in HHAF reclaiming said animal.

3. All animals are to be made and kept current on vaccines and other necessary medical needs as

recommended by their vet. Happy Hills will provide such vaccines at no cost to foster family.

4. No refund or substitution shall be given for any animal that has been lost, stolen, killed, or

any illness or injury that has been caused by neglect or abuse and/or breach of contract.

5. Fosterer agrees to exercise due and reasonable care of said pet. The pet is to be fed and watered

properly and regularly and is to be housed in clean, safe quarters. Pet is to be kept in a manner

so as not to be endangering itself or any other person or animal.

6. Proper restraint is to be provided and maintained to insure the pet stays on your property.

Animals fostered from HHAF are intended to be part of your family. They are not your

neighbors' responsibility. You are liable for any damages to other people's properties that is

done by your animal. Cats are to be kept indoors only.

7. HHAF reserves the right to reclaim said animal if it is found at large or is not in compliance

with our requirements in addition to local animal control laws.

8. Fosterer authorizes HHAF, in its sole discretion, to determine whether or not said pet will be

reclaimed and fostering terminated if conditions of this contract are not upheld. No substitution will

be given if reclaimed for failure to comply.

9. Fosterer agrees to permit prospective adopters to meet animal in their home, take animal to HHAF, or transport animal to adoption fairs as needed to help find the foster animal a permanent home.

10. Fostering agreement may be terminated at any time by HHAF or fosterer and animal will be returned to HHAF immediately following such termination. Fosterer agrees to pay legal or other fees as needed in the event that HHAF must reclaim animal due to fosterer’s failure to comply with contract. (including hourly wages for HHAF representative for time spent trying to resolve dispute)

11. Fosterer will be responsible for providing food, shelter, and flea control for pet. Dogs may be brought to HHAF monthly for heartworm preventative at HHAF expense or fosterer may provide heartworm preventative at their expense. Pets must be returned to HHAF for any non-emergency vet care or medications needed. Any exceptions must be pre-approved in writing. Any non-approved vet expenses are the responsibility of the fosterer, and will be treated as donations, with a tax receipt provided upon request.

12. The fosterer will meet the terms of this contract, and obtain written permission from HHAF, before

relinquishing fostering to a second party. Fosterer agrees not to sell, give, or use fostered pet for

any purpose other than a household pet.

HHAF agrees to provide copy of contract and medical information available for said pet.
