OUT OF STATE ADOPTION APPLICATION
Please have your vet fax us a reference on letterhead (can be as simple as one sentence that states your past/present animals are up to date on their shots, spayed/neutered, and that you get them vet care when needed).
You will also be required to provide proof that you own your home or that you where you rent permits you to have pets of the type you have applied to adopt. This may be faxed along with your vet reference if you like.
Fax: (336) 622-3677
Applications will be processed as soon as vet reference comes in.
Because of the high number of applications, and the fact that each one must be printed and followed up by our staff, we are now requesting a $5 donation to help cover the supply and staff costs. Click

Terms
You must be 18 yrs. or older or have legal guardian sign for adoption (certified by notary public). The legal guardian will be responsible for conditions of adoption to be met.
Submitting application does not guarantee adoption. HHAF personnel are authorized to make final determination after consultation and at their discretion. There may be a waiting period between submission of application and adoption being granted, in order to verify information. We reserve the right to ask to see where the animal will be living, do follow up checks on veterinarian visits and reclaim the animal if adoption agreement has not been met within given time unless documentation from veterinarian is given that it is not in the pet's best interest for medical reasons.
Only altered or to be altered animals will be adopted from HHAF. No animals are to be bred or allowed to breed, as this may result in HHAF reclaiming animal. All animals are to be made and kept current on vaccines and other necessary medical needs as recommended by their veterinarian.
Proper restraint is to be provided to maintain animal on your property. Animals adopted from HHAF are intended to be a part of your family and kept in a clean and safe environment. HHAF reserves the right to reclaim animal if it is found at large or is not in compliance with our requirements in addition to your city/state local animal control laws.
All animals will be spayed or neutered before being adopted out of state. Spay/neuter cost is included in adoption fees of $170 for dogs, $100 for female cats, and $75 for male cats. Microchipping is included in adoption fee for dogs, required for cats at $20.
There is an additional $50 charge for out of state adoptions to cover long distance calls, airport trips, etc. Full medical records will be provided with adopted animal. For small animals, or any animal flying by air, there may be a need for a crate for which the adopter must pay for up front. The crate goes with the animal and remains the property of the adopter. Please see

NO ANIMAL WILL BE SHIPPED BY AIR DURING THE DAY IN THE HOT SUMMER MONTHS! (Most airlines won't ship May-September anyway unless it is a night flight.)
